· Prescription management
· Chronic illness management

Registration: $150, capped at $200 per household
Monthly Price: Age 20 and up, $65 monthly
0-19 living at home, $15 monthly
CoPay: $50 for in person visit
Rules: Must reside 50+ miles away from the clinic
Term: 12 months, paid monthly
15% discount for paying a year at a time (non refundable).
Policies
· $50 no show fee for cancellations less than 24 hours, card will be automatically charged
· Labs, medications and services are charged to card on file when provided, ordered or filled
· Labs must be completed prior to initial visit for weight loss and hormone replacement
· All terms are as above and no refunds/early cancellations. Memberships renew at the new rates for the appropriate terms and run for the listed term.
We do not accept insurance, but suggest you carry a major medical or share plan.
Many of the share plans such as Zion, Liberty, and Christian Medi-Share, offer discounts for DPC members.
We are sorry, we cannot accept Medicaid patients.
