Hormone Replacement ThErapy

Hormone replacement therapy (HRT) involves taking medication to replace the hormones that are no longer produced by the body or are produced in lower amounts. It's important to note that HRT has both potential benefits and risks. Make a consultation appointment to determine if HRT is the right option for you.

Hormone Replacement Only Membership

Benefits:

  •       Care provided by Certified Menopause Society Provider

  • Experienced Men’s Health Provider

  •       Discounted Labs (required labs 2x a year for women complete female hormone panel, CMP, CBC, TSH and A1C, 2 x a year and 2x a year for men complete male hormone panel, CMP, CBC, TSH, PSA and A1C+ Q8 weeks CBC)

  •        Discounted Medications

  •       Texting, e-mail access responses M-F 10-6

Registration:       $100 includes initial visit 60 minutes, labs must be completed first

Monthly Fee:     $45 includes one 30 minute telehealth or in person visit per calendar month

Term:                   3 months

Copay:                 $50 per 30 minute visit (after included visits)

 

Policies

·        $50 no show fee for cancellations less than 24 hours, card will be automatically charged

·        Labs, medications and services are charged to card on file when provided, ordered or filled

·        Labs must be completed prior to initial visit for weight loss and hormone replacement

·        All terms are as above and no refunds/early cancellations.  Memberships renew at the new rates for the appropriate terms and run for the listed term.
