We do not accept insurance, but suggest you carry a major medical or share plan.
Many of the share plans such as Zion, Liberty, and Christian Medi-Share, offer discounts for DPC members. We cannot accept medicare.

Individual Rates

Age 19-up: $95 per month

  • Age 0-19: $25 per month (under the family plan rates)

  • Activation Fee: $150 one time only

  • 15% discount on annual fees if paid in full ($969 annually vs. $1,140)

Family Rates

  • Regular pricing for Adult Family Members, family pricing capped at $225 per family

  • $25 per month for children living at home up to age 19    

  • Activation fee is capped at $200 per family. A family consists of individuals living at the same address as their minor children

Telehealth Only

  • Registration: $150, capped at $200 per household

  • Monthly Price: Age 20 and up, $75 monthly

    • 0-19 living at home, $15 monthly

  • In-office Visits: Additional $50 fee for an in person visit

  • Rules: Must reside 50+ miles away from the clinic

  • Term: 12 months, paid monthly
